We Will Work with You To Create the Right Care Plan for Your Specific Needs

Care plans are a critical component of providing quality care. They clarify the roles of support and care workers and allow the care team to tailor the degree and types of support for different clients based on their specific needs.

We follow a care-receiver-first approach, prioritizing the receiver's needs as we know that a one-size-fits-all care plan will not work for everyone. Keeping in mind the unique challenges every patient faces, we develop a care plan that includes home care services that are most valuable to the care receiver.

How Our Personal Care Consultation Can Benefit You

Our personal care consultation helps us serve you better by suggesting care plans that ensure improved outcomes.

As part of our care consultation process, we will:

  • Assess your care needs: Developing a care plan begins with gathering information and addressing current concerns. Therefore, we carefully assess each area of a care receiver's daily life, identifying their unique issues and developing a customized strategy to meet their care needs.
  • Discuss your current caregiving situation: After assessing your needs, we identify the gaps in current caregiving situations to understand what more can be done to address your concerns in the best way possible.
  • Review Activities of Daily Living (ADLs): Next, we evaluate and review activities of daily living (ADLs) such as bathing, dressing, cooking, and transferring in and out of a chair or bed. The objective is to identify the activities the client can perform without assistance and those for which they need help.
  • Identify shortcomings or concerns: The existing caregiving system is evaluated for shortcomings and concerns addressed and prioritized in our newly developed plan.
  • Conduct home safety checks: To ensure the safety of the care receiver and the caregiver, we conduct home safety checks in which we look for any potential safety risks, such as trip hazards, a lack of grab bars in moist areas, misuse of extension cords, or space heaters, etc.
  • Perform social assessment: As part of the consultation, we look for signs of any physical or emotional issues arising from loneliness and isolation.
  • Design care coordination plan: We discuss existing or potential health concerns, medications, and other matters and design a suitable care coordination plan.
  • Provide recommendations based on consultation: Once we have assessed your condition and discussed your concerns, we will recommend a plan best suited to your needs.
